Home Health Care is for those individuals who may require assistance during an illness or after surgery. Services are provided in the comfort and privacy of one’s home to those persons who are in need of nursing, therapy services to include: Physical, Occupational, and Speech Therapy. MSW for community resources and HCA for personal care and/or assistance with the activities of daily living.

How do I qualify for Home Health services?

There are three ways of obtaining a referral for Home Health: Your Primary Care Physician can order it, you can contact our Admissions Team at 303-955-0550 or you may also click the “Request More Information” link to automatically have your information sent to our Admissions Team. You must have a skilled Medical need and be homebound. This means that it takes great effort for you to leave the home to receive these services.
